The fillable one lets you type straight onto the page and print a clean copy, which matters when a nurse has to read it fast. The other is blank, for filling in with a pen.
Do these two things now, while you are here
Fill it in and print four or five copies. Keep the spares in the pocket inside the front cover of whatever you carry. You will hand one over every time someone new gets involved.
Photograph the filled page with your phone. The paper copy will be at home on the day you need it somewhere else.
